7:04 a.m. Craig arrived. Joyce asleep with oxygen in place. Blood oxygen 97% Pulse 74
7:30 a.m. Joyce heads to the bathroom (No data on urine/bowel movement or incontinence issues)
7:46 a.m. Joyce returns from bathroom.
8:36 a.m. Dusty arrived. Joyce asleep hunched forward with oxygen in place. BP 121/69 Pulse 74 Oxygen @ 94% Blood glucose @ 163. Bathroom trip declined. claimed she already went (remembered) Evidence of early bladder release. Dusty sanitized bathroom floor.
8:48 a.m. Breakfast (cream of wheat with equal, banana, dices peaches, cheese round) Dusty also set mouse traps up in pantry and upstairs.
9:47 a.m. Breakfast paused, Holly (Kindred PT) arrived to do start of care paperwork. Joyce fairly responsive. Recommendation family consider having vision and hearing checked. Reviewed dental history- Joyce claimed she doesn't have dentures. Slight pain reported to Holly- day & night - unable to rate pain level (maybe a 2?) Reviewed injuries/ pain that reoccurs. No numbness or tingling reported. Reviewed family care plan. Reviewed sleeping & toileting habits. Reviewed safety equipment. Reviewed medications. Mental assessment - basic cognitive, handedness, date (cheated). Reviewed meals, fluid in take. Reviewed June 2020 fall (Joyce remembers) Weight reviewed. Depression/ Anxiety reviewed. Why taken of potassium? Usually put on a potassium supplement when put on a diuretic. Need a copy of advanced directive (picture emailed to Kindred okay). DNR? Contact & Craig as PoA confirmed. Joyce goal - mobility, go outside & play on patio. Confirmed Joyce should only go outside with close supervision. Joyce knew all sons vitals collected, some difficulty in getting readings BP 132/68. Some swelling in legs - confirmed compression stockings and leg elevation. Mobility and balance checked. Oxygen 86% - 90% (90% after breathing exercise). 88% after additional activity. Pressure sore on buttock checked, logged. PT 1/week for 8 weeks, Marshall and Jay again, work on strength, balance, endurance, stepping up and down. Confirmed OT will be coming out, too. Medicare paying for PT. Reading test passed. Important to change Joyce's positions in the recliner.
11:11 a.m. Holly departed.
11:12 a.m. Bathroom. Urinary incontinence. New diaper ( No pad in old one), New pad. Urine passed.
11:35 a.m. Morning medication + cranberry supplement with nutritional shake. Breakfast finished. Nap.
2:44 p.m. Enrichment activity- Folding laundry. Joyce very excited for this.
5:58 p.m. Bathroom. Urinary incontinence. Early bladder release. New diaper and pad. Remaining urine passed. Bathroom floor and toilet riser sanitized. PT walk. Returned to recliner 6:14 p.m.
6:47 p.m. BP 112/60 Pulse 66 Oxygen @ 96% Blood glucose @ 116
6:52 p.m. Dinner (Cheese manicotti, applesauce, and butterscotch pudding) Finished by 7:33 p.m.
7:36 p.m. Evening medication with nutritional shake. Began napping.
10:25 p.m. Bathroom. Dry. Urine passed. PT walk declined.
10:37 p.m. Oxygen @ 92% Pulse 66
10:40 p.m. Ready for bed.
