General Update #291 - Thursday, January 28, 2021 (Updated)

7:05 a.m. - Craig noted that Joyce was making her first bathroom trip of the day. Joyce appears to be in a much better state of mind (less general confusion). Returned to recliner by 7:15 a.m.

7:58 a.m. - Jess arrived to farmhouse. Joyce found dozing in her recliner without oxygen line in place.

7:59 a.m. - BP 159/83, pulse77. BGL @ 145.

8:30 a.m. - Nutritional shake and morning medication.

8:35 p.m. - Breakfast (1 slice egg in toast with banana). Breakfast finished by 9:07 a.m.

Joyce very tired. Joyce informed Jess she was "gone" one of the two days Jess was not at the farmhouse. Jess asked for clarification. Joyce took some time to articulate her meaning with prompting from Jess - Joyce was trying to say that she was not "there mentally."

9:30 a.m. - Bathroom trip. Urinary incontinence. New diaper and diaper pad insert. Remainder of urine passed. Barrier cream applied. Scab has formed on her right butt cheek. Returned to recliner by 9:40 a.m.

Joyce given fresh blankets by Jess.

9:47 a.m. - Joyce setup to nap in her recliner. Joyce still very tired from this morning.

Dusty arrived to farmhouse ~12:30 p.m., found Joyce asleep in her recliner with oxygen line in place, hunched forward over her knees. Joyce had turned her television off.

12:38 p.m. - BP 117/61, pulse 70. BGL @ 162. Lunch refused, bathroom trip refused.

Joyce's fingernails cleaned and trimmed.

Bathroom trip refused. Lunch refused.

1:09 p.m. - Lunch (mac & cheese, V8 juice, watermelon spears). Tramadol and 2 Tylenol. Joyce continually fell asleep while eating, stared off into space, or "stalled out" while chewing and sat holding her head or leaning sideways in her recliner. Dusty had to keep prompting her to stay awake and eat. Joyce refused to relinquish tray, stating she wanted to keep eating. 

Joyce became emotional about not having water in her glass and only having V8 juice to drink with her meal (she had drained her water glass empty after it had been refilled in several gulps after getting her lunch tray and Dusty hadn't noticed while loading the dishwasher).

~2 p.m. Craig stopped in to visit and provided Dusty with update on Joyce from last evening. Craig and Carol had found Joyce in her chair with lunch tray still on her lap and fairly confused. Joyce struggled with confusion throughout the evening care hours.

Lunch abandoned by 2:26 p.m. but Joyce kept a spear of watermelon...only to throw it into the garbage a few minutes later.

2:42 p.m. - Bathroom trip. Urinary incontinence. Early bladder release. Remainder of urine passed. Bathroom toilet riser, floor, and sink sanitized. Joyce was cleaned up with disposable washcloth. PT walk to living room before returning to recliner by ~2:58 p.m.

Dusty departing farmhouse ~3:05 p.m. after getting Joyce settled into recliner to nap.

5:10 p.m. - Bathroom trip.

6 p.m. - Dinner (Panda Express - noodles, teriyaki chicken).

8 p.m. - Bathroom.

8:25 p.m. - Bed - relaxing.

Joyce very lethargic today and at times unable to focus on conversation or articulate information effectively to family caregiver.

Joyce is able to stand/sit easily and claims to not be hindered by pain. Joyce was able to walk under her own power using her walker at a quick and stable pace - no balance issues today when moving for toileting.
